What To Bring For Your Visit   

  • Vaccination record (your doctor can fax records to 614 645-1784)
  • Insurance Card
  • I.D Card (parent's I.D. if insurance is in parent's name)
  • Any custody or guardianship papers

Services Others Offer (not by Immunization Clinic) 

  • Travel Immunizations- Columbus Public Health does not provide travel immunizations.  Information about which vaccines are recommended for various destinations can be obtained by calling Ohio Department of Health at 614-466-4643.  Here is a list that can help you find travel vaccines in Central Ohio.
  • Physical Examinations for Work or School- Columbus Public Health does not offer physical examinations.
  • Tuberculosis (TB) Testing - If you plan to get vaccines AND a TB test, it is best to get the TB test done first.  This can prevent the long wait (4-6 weeks) that can be required before a TB test can be done after certain vaccines have been given.  TB testing and locations.

  4. Insurance: These plans are accepted (plans may not cover all vaccines)

Insurance cards must be presented at time of service, and will be accepted if the patient is eligible for coverage. To use an insurance card you must present photo ID. If you have immunization coverage with another insurance company, check with them for an in-network place to get vaccines, or visit us and ask for a copy of the Fee Ticket which you can submit to your insurance company for reimbursement.
